Locate Birth and Death Records

The Jefferson County Clerk’s Office is not the local registrar for birth and death records filed after 1966. All requests for birth or death certificates can be obtained by submitting an Application completed by a qualified applicant, proof of identification, Affidavit of Personal Knowledge and payment.

ORDER ONLINE FOR IN OFFICE PICK-UP

  1. You will receive an Order Confirmation Number with an option to email your confirmation to yourself. In order to expedite the process, please have this Order # available when visiting our office.
  2. Come to our Main Office on Pearl Street within five (5) business days.
  3. Bring a Valid form of Identification, see below link.

ORDER ONLINE FOR RETURN BY MAIL [USPS or FEDEX]

  1. You will receive an Order Confirmation Number with an option to email your confirmation to yourself.
  2. Email, Mail or Fax your Order Confirmation Number, valid photo ID and the Notarized Proof of Identification Form. Email: Filing@jeffcotx.us
  3. Email and Fax Order, don’t forget to provide a phone number to make contact for payment via Credit Card, a $2.50 credit card processing fee applies. Do Not Mail Cash.
  4. Please allow 7-10 business days, from the date we receive the request, for processing.

IN PERSON


Applications are available to complete when you visit the clerk’s office. Applications will be processed during normal business hours, 8:00 AM to 4:30 PM, Monday through Friday at our Main office on Pearl Street only.
In most cases, you will get your certificate the same day, but some applications require 24 hours or more to process.

BY MAIL


Mail-in orders can be processed with standard or expedited service. When requesting a copy of birth or death certificate by mail complete the APPLICATION FOR BIRTH OR DEATH CERTIFICATE and a Notarized Proof of Identification form (which must be signed and notarized) include a copy of your valid identification (see below link) along with your payment. Follow the submission instruction provided on the instruction page.

Who can REQUEST records?


Qualified Applicants: The registrant, or immediate family member either by blood, marriage or adoption, his or her guardian, or his or her legal agent or representative. Local, state and federal law enforcement or governmental agencies and other persons may be designated as properly qualified applicants by demonstrating a direct and tangible interest in the record when the information in the record is necessary to implement a statutory provision or to protect a personal legal property right. Immediate family member-- The registrant, his or her guardian, or the children, spouses, parents, siblings, or grandparents of the registrant. TITLE 25, RULE §181.1 Texas Administrative Code

For birth and death, certificates after 1966 please contact the City Health Departments in the city where the birth or death occurred or contact the Bureau of Vital Statistics in Austin.
